1. Acknowledgment of Risk

I, the undersigned participant (or parent/legal guardian if the participant is under 18), acknowledge that participation in activities at Stadium Sports LLC, including but not limited to soccer training, leagues, tournaments, private sessions, open play, and events, involves inherent risks. These risks may include, but are not limited to, bodily injury, falls, collisions, equipment failure, and exposure to communicable diseases, including COVID-19.

I understand that these risks can result in serious injury, permanent disability, or even death, and I voluntarily assume all risks associated with my participation in activities at Stadium Sports LLC.

2. Release and Waiver of Liability

In consideration of being allowed to participate in activities at Stadium Sports LLC, I, on behalf of myself, my heirs, personal representatives, and assigns, release, waive, discharge, and hold harmless Stadium Sports LLC, its owners, officers, employees, trainers, volunteers, sponsors, and affiliated organizations from any and all claims, liabilities, demands, actions, or causes of action, including negligence, that may arise from my participation in any Stadium Sports LLC activities.

This waiver extends to injuries, damages, or losses caused by the ordinary negligence of Stadium Sports LLC or any of its representatives.

3. Medical Treatment Authorization

I authorize Stadium Sports LLC staff to provide first aid and, if necessary, arrange emergency medical treatment for me (or my child). I understand that I am responsible for all medical costs incurred due to any injury sustained while participating in Stadium Sports LLC activities.

4. Code of Conduct & Facility Rules

I agree to abide by all rules and policies of Stadium Sports LLC. I understand that failure to follow facility rules or engaging in reckless, dangerous, or unsportsmanlike behavior may result in my removal from the premises without refund.

5. Photography & Media Release

I grant Stadium Sports LLC the right to use photographs, videos, and other media featuring me (or my child) for promotional and marketing purposes, including but not limited to social media, websites, and advertisements. If I wish to opt out, I will notify Stadium Sports LLC in writing.

6. Parent/Guardian Agreement (For Minors)

If the participant is under 18 years old, this waiver must be signed by a parent or legal guardian. The undersigned parent/guardian acknowledges and agrees to all terms on behalf of the minor.
